Who we are
Back to Health Chiropractic Pty Ltd provides chiropractic and remedial massage services at our Toowoomba and Warwick practices. This policy explains how we handle personal information collected through this website, backtohealthchiro.com.au.
As a health service provider, we are bound by the Privacy Act 1988 (Cth) and the 13 Australian Privacy Principles, regardless of our size or turnover.
What we collect through this website
Our enquiry form collects:
- Your name
- Your phone number
- Your email address
- Which practice your enquiry is for, Toowoomba or Warwick
- Anything you write in the message box
Because this form is used to ask about and book chiropractic and remedial massage appointments, anything you tell us about your symptoms, injuries or health is health information. Health information is treated as sensitive information under the Privacy Act and we handle it with the higher level of protection that requires.
You do not have to give us any of this. If you would rather not use the form, you are welcome to phone either practice instead. If you do not give us contact details, we will not be able to reply to you.
Why we collect it
Only to respond to your enquiry, arrange or change an appointment, and provide our services to you. We do not sell your information, and we do not use it for advertising.
How we hold it
When you send the form, two things happen:
- It is emailed to the relevant practice.
- A copy is saved in this website's database so an enquiry is not lost if an email fails.
We take reasonable steps to protect that information.
Who else is involved
Brevo delivers our enquiry emails. Brevo is based in the European Union and stores data on servers in France, Germany and Belgium, so your enquiry is disclosed overseas.
Google Maps is embedded on our location pages to show where we are. If a map loads, your IP address is received by Google in the United States. You can view a location page without interacting with the map.
Accessing or correcting your information
You can ask for a copy of the personal information we hold about you, or ask us to correct it.
Changes
We have the right to make changes to the privacy policy at any time.
If we change this policy we will publish the new version here with an updated version number and date.
